Summary

연구 필요성

Crohn’s disease shows considerable heterogeneity in disease progression and treatment response. Bulk transcriptomic studies cannot resolve cell type-specific changes, necessitating single-cell approaches to better understand disease mechanisms.

연구 목적

To characterize cellular composition and transcriptional changes in the ileal mucosa of treatment-naïve and treated Crohn’s disease patients using single-cell RNA sequencing

실험 방법

Terminal Ileal biopsies from 27 subjects (from controls, treatment-naïve CD patients, and established CD) were analyzed using 10x Genomics single-cell RNA sequencing. Analyzed for cell proportions and differential gene expression analysis, and GO enrichment analysis were performed

주요 결과

  • Treatment-naïve Crohn’s disease exhibited substantial epithelial remodeling characterized by alterations in enterocytes, goblet cells, tuft cells, BEST4+ cells, and M cells.
  • Immune-cell abundance showed relatively minor changes.
  • Both epithelial and immune compartments displayed significant transcriptional reprogramming, particularly involving antigen-presentation and inflammatory pathways.
  • Remission and refractory patients were distinguished primarily by transcriptional differences rather than cellular composition.

Take home message

The study suggests that Crohn’s disease is driven more by changes in cellular function and transcriptional activity than by large shifts in cell abundance

Points helpful for my research

This study supports the use of scRNA-seq for patient stratification and highlights the importance of integrating cellular composition with transcriptional states. The findings are particularly relevant for identifying immune and epithelial signatures associated with disease activity, remission, and treatment resistance.
